Chronic nonspherocytic haemolytic anaemia is characterized by persistent haemolysis in the absence of demonstrable autoantibodies, abnormal haemoglobins, alterations in erythrocyte morphology, or other obvious findings suggesting the etiology. Although patients with such presentations are not frequently encountered, they require careful evaluation, both to establish the cause of their anaemia and in an effort to avoid unnecessary or potentially harmful therapy. Making the specific diagnosis is a matter of ruling out possible causes of life-long haemolysis while keeping in mind the myriad etiologies of anaemia-including erythrocyte enzyme deficiencies.
