Direct endoscopic necrosectomy is a cumbersome and time-consuming procedure, that can be necessary in cases of infected walled-off pancreatic necrosis not responding to drainage only as a step-up approach after EUS-guided drainage OR as a primary treatment in a case of high necrotic solid content. Until now, DEN has been performed with non-dedicated devices: common polypectomy snares, caps, Dormia baskets and retrieval nets. In some cases, a mechanical lithotriptor was used for fragmentation of large necrotic fragments. In this way DEN requires multiple, long-lasting sessions to achieve adequate clearance of solid necrotic content. The multiple endoscopic sessions result in prolonged hospital stay, increased costs and have potential consequences for patients who must undergo multiple interventions under deep sedation or anesthesia. Necrolit was born from a constant research and development activity in order to develop a dedicated and specific endoscopic device for Direct Endoscopic Necrosectomy. The double lumen catheter allows two devices to coexist inside, each of them with its own reference handle. 1 A special monofilament loop with a diameter of 0.90 mm, ultra stiff ends with a rounded tip to avoid the traumaticity of it during use. The internal section of the loop has the peculiarity of being flat. This results in a greater shape memory and a superior synergistic ability to cut and grip the necrotized tissue. 2 A particular container of equivalent width and length, with nitinol filaments with variable flexibility that give perfect retrieval capacity.
