The “Failure Mode and Effects Analysis” (FMEA) is a method to identifying all possible failures in a design or an automated process. This method is less suitable if operator errors play an essential role or detecting combinations of failures. Once the failure modes are identified, they are prioritised according to the seriousness of their consequences and frequency of occurrence. FMEA studies are typically used for vessel DP systems, automatic ballast systems and unmanned engine rooms and can be a requirement for the vessel's assurance.
